Job Summary
Hospice Clinical Liaison role involves promoting Compassus hospice services, explaining benefits to patients and families, obtaining required forms, coordinating admissions from hospital to home hospice, coordinating transportation and supplies, and maintaining relationships with community, hospital, and facility education representatives. The position requires an active RN license, a bachelor’s degree in nursing is highly preferred, 2–3 years of related healthcare experience, and strong communication skills to educate hospitals and facilities about hospice care, eligibility assessment, and the admissions process. The role emphasizes coordinating patient discharge, aligning with care needs, and supporting ongoing care delivery within a hospice program.
Required Qualifications
- RN license required
- Bachelor's degree in Nursing highly preferred
- 2 to 3 years of related experience required
- Hospital and/or long-term care clinical experience highly preferred
- Experience in healthcare marketing preferred
