Job Summary
Coordinate and oversee direct and indirect patient services provided by clinical personnel while ensuring Medicare, State, and ACHC regulations are followed. Provide supervision and guidance to nursing and health aide staff, performing chart audits and appraising performance during probation and annually. Identify and evaluate patient needs by reviewing intake screening forms before assignment, assist with orientation of new staff, and see patients in the home setting as needed. Gather data for QAPI activities and maintain adequate medical supplies. This role requires full-time employment in Oklahoma City with travel to patient homes and physical dexterity for assessments.
Required Qualifications
- Graduate of an accredited School of Nursing
- Registered Nurse licensed in the state of OK
- Bachelor of Science in nursing
- Two (2) years combined public health nursing and progressive professional responsivities in public health nursing
- Three (3) years combined public nursing and progressive professional responsibilities in public health nursing
- Minimum of 2 years nursing experience in home hospice or skilled home care
- Ability to market and deal tactfully with customers and the community
- Knowledge of business management, governmental regulations and ACHC standards
- Ability to communicate well, both verbally and in writing
- Ability to demonstrate self-confidence and positive attitude toward self and others
- Commitment and enthusiasm to goal achievement
- Travel is required to and from patient's home when supervision of care is scheduled
- The ability to access patients' homes which may not be routinely wheelchair accessible
- Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care
- Must be able to lift and/or carry up to 30 pounds
- Must be able to bend, stoop, reach and move patient weight up to 250 pounds
