Job Summary
Conduct spiritual assessments for patients, families, and caregivers to ensure timely service delivery and facilitate individualized care plans. Provide direct spiritual care, serve as a liaison to community chaplains, and document all services for quality assurance and policy review. Participate in interdisciplinary meetings, provide consultation on spiritual care topics, and recruit community volunteers and clergy. Plan memorial services and assist in supervising spiritual care volunteers when assigned. Maintain records of activities and actively participate in quality assessment and performance improvement teams.
Required Qualifications
- Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree)
- Experience working with death and dying, individuals/family/caregiver
- Ability to work as member of interdisciplinary group
- Comfort in an interfaith setting
- Ability to accept different lifestyles, cultures, beliefs, and values
- Ability to network with community clergy and congregations
- Knowledge of and commitment to hospice philosophy of care
- Licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
