Job Summary
Hospice Chaplain needed to provide in-home spiritual care within Pathways Healthcare’s hospice program. Responsibilities include spiritual assessment of patients and families, development of individualized plans of care, direct spiritual care provision, liaison with community chaplains and spiritual counselors, maintaining utilization records for quality assurance, documenting services and communications, attending patient care conferences as part of the interdisciplinary team, offering consultation, education and support to the interdisciplinary group, and assisting with funeral/memorial services and supervision of spiritual care volunteers. Requires graduate theological degree (BD/MDiv or equivalent), experience with death and dying, ability to work in an interdisciplinary team, comfort in an interfaith setting, and valid driving to make home visits. Per Visit compensation with mileage reimbursement; flexible schedule and benefits package are provided.
Required Qualifications
- Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree)
- Experience working with death and dying, individuals/family/caregiver
- Ability to work as member of interdisciplinary group
- Comfort in an interfaith setting
- Ability to accept different lifestyles, cultures, beliefs, and values
- Ability to network with community clergy and congregations
- Knowledge of and commitment to hospice philosophy of care
- If making home visits, must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
- Hospice/experience preferred
