Hospice Chaplain

Job Summary
Hospice Chaplain providing direct spiritual care to patients and families in home-hospice settings. Responsibilities include conducting spiritual assessments, developing and contributing to individualized care plans, serving as liaison with community chaplains and spiritual counselors, maintaining records of spiritual care services for QA and program development, attending patient care conferences as part of the interdisciplinary team, offering consultation and education to the team, and assisting in supervision of spiritual care volunteers. Position involves performing funeral or memorial services as requested and participating in quality assessment and performance improvement activities. Requires graduate theological degree, experience with death and dying, ability to collaborate with an interdisciplinary team, comfort with interfaith settings, openness to diverse beliefs, and valid driver with insured vehicle for home visits. Per visit compensation, flexible schedule, and benefits package available.
Required Qualifications
- Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree)
- Experience working with death and dying, individuals/family/caregiver
- Ability to work as member of interdisciplinary group
- Comfort in an interfaith setting
- Ability to accept different lifestyles, cultures, beliefs, and values
- Ability to network with community clergy and congregations
- Knowledge of and commitment to hospice philosophy of care
- If making home visits, must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
- Hospice/experience preferred
