Job Summary
Conduct comprehensive spiritual assessments of patient and family units to determine needs and dynamics, documenting findings consistent with hospice standards. Formulate plans of care for patients, families, and caregivers as a member of the interdisciplinary team, bringing psychosocial, spiritual, physical, and financial conditions to team conferences. Provide individual and family spiritual support at regular intervals while coordinating care with the hospice team and identified clergy. Maintain accurate patient records, care plans, and family contact logs, adhering to HIPAA confidentiality requirements. Participate in quality assessment and performance improvement programs, attend required in-services education, and accept assigned agency-wide initiatives.
Required Qualifications
- Commitment to serving the needs of the terminally ill
- Ordination or other spiritual/pastoral training
- Valid Driver's License with a reliable and insured vehicle
Desired Qualifications
- Hospice, hospital chaplaincy and/or experience working with terminally ill individuals and their families
