Hospice Chaplain

Job Summary
Conduct comprehensive spiritual assessments, develop individualized care plans, and provide counseling, prayer, rituals, and compassionate presence for hospice patients, families, and bereaved individuals. Collaborate with physicians, nurses, social workers, and volunteers within the Interdisciplinary Group to coordinate care, while documenting all interventions in the electronic medical record per Medicare and Massachusetts regulations. Respond to urgent spiritual needs during evenings, weekends, or holidays as assigned, and offer grief support and advance care planning assistance. Maintain strict confidentiality and participate in care conferences, continuing education, and quality improvement activities.
Required Qualifications
- Graduate degree in Divinity, Theology, Pastoral Care, Religious Studies, or a closely related field from an accredited institution
- Equivalent theological education recognized by the applicant's faith tradition
- Qualifications consistent with the standards of the applicant's religious denomination or faith group to provide professional spiritual care
- Minimum of one (1) year of experience providing spiritual or pastoral care in healthcare, hospice, hospital, palliative care, long-term care, or similar setting
