Job Summary
Provide direct personal care to terminally ill patients, assisting with hygiene, grooming, toileting, safe mobility, meal preparation, and light housekeeping. Observe and report changes in patient conditions while offering emotional support to patients and families. Maintain accurate visit documentation, attend team meetings for Quality Improvement activities, and strictly follow infection control and safety protocols. This safety-sensitive role requires an active CNA license, current CPR certification, and a driver's license. Six months of direct patient care experience is required, with comfort working in home settings, hospice facilities, or long-term care environments. Required Qualifications
- Active CNA license in the state of employment
- Current CPR certification
- Driver's license and reliable vehicle with insurance
- Six (6) months of direct patient care experience
- Comfortable working in home settings, hospice facilities, or long-term care environments
- Demonstrated compassion, patience, and ability to handle emotional situations
- Willingness to work independently and adapt to changing patient needs

Desired Qualifications
