Job Summary
Deliver direct personal care to terminally ill patients by assisting with hygiene, grooming, toileting, safe mobility, meal preparation, and light housekeeping. Observe and report changes in patient conditions while offering emotional support to patients and families. Maintain accurate visit documentation, attend team meetings, and practice infection control protocols. This role requires an active CNA license, CPR certification, driver's license, and six months of direct patient care experience. Heartland Hospice, part of Gentiva, provides compassionate end-of-life care across nearly 600 locations nationwide.
Required Qualifications
- Active CNA license in the state of employment
- Current CPR certification
- Driver's license and reliable vehicle with insurance
- Six (6) months of direct patient care experience
- Comfortable working in home settings, hospice facilities, or long-term care environments
- Demonstrated compassion, patience, and ability to handle emotional situations
- Willingness to work independently and adapt to changing patient needs
- This is a safety-sensitive position
- Employee must meet minimum requirements to be eligible for benefits

Desired Qualifications
- Experience in home health, hospice, palliative care, or oncology
- Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes
- Exposure to end-of-life care, wound care, or admissions support
