Job Summary
Deliver empathetic and compassionate care to patients in their homes or facilities, assisting with activities of daily living, pain management, and symptom control. Provide emotional support and companionship to patients and their families while coordinating care with the interdisciplinary hospice team. Maintain accurate patient records and documentation to ensure a peaceful and comfortable environment. Act as an advocate for patients and their families throughout their journey. Valid CNA certification, driver's license, and state auto insurance are required. Competitive compensation and a robust benefits package are offered.
