Job Summary
Provide end-of-life patient care as a Hospice CNA, including assisting with personal hygiene, monitoring vital signs, administering medications as directed, and offering companionship and emotional support. Assist with mobility and positioning to enhance comfort, maintain safety, and prevent bedsores. Document care with time-stamped notes for insurance purposes and coordinate with the supervising nurse, healthcare teams, and patients' families to ensure continuity of care. Adhere to infection control and safety procedures; collaborate with family members and healthcare professionals; maintain a clean and safe patient environment; and perform other duties as assigned. Requires a valid CNA license; 1 year of hospice or palliative care experience preferred. Strong communication and interpersonal skills, compassion, and the ability to manage multiple tasks in a fast-paced setting. Physical requirements include lifting up to 50 lbs and ability to navigate home environments. Schedule: Full-Time, Weekdays, 8:30 AM - 5:00 PM; Phoenix, AZ.
