Job Summary
Provide hands-on personal care to hospice patients, including bathing, grooming, dressing, repositioning, and ADL support. Assist with maintaining a safe, clean, and comfortable environment in the home or facility setting. Observe and report changes in the patient's condition to the RN Case Manager. Offer emotional support to patients and families with sensitivity and respect. Document care accurately and participate in team communication to ensure coordinated, high-quality care. Travel to patient homes, facilities, and communities within the assigned coverage area. This position offers flexible PRN scheduling and competitive hourly pay rates for a dedicated team providing compassionate end-of-life care across the Albuquerque region.
Required Qualifications
- Current New Mexico CNA certification
- Reliable transportation
- Valid driver's license
- Ability to work independently in home settings with professionalism and compassion
- Strong communication skills
- Time-management skills
- Interpersonal skills
- Ability to comfort supporting patients and families during end-of-life transitions
- Must be available for travel to patient homes, facilities, and communities within assigned coverage area
Desired Qualifications
- At least six months direct patient care experience
- Hospice experience
- Home health experience
