Job Summary
Deliver compassionate, patient-centered care in a hospice setting by assisting patients with activities of daily living and reporting condition changes to support comfort and quality of life. Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand the hospice journey. Respond promptly to patient needs with empathy, accurately document care provided in the EMR, and work independently while traveling to patient homes within your assigned territory. Maintain a current driver's license and spend approximately 20% of your day driving to/from locations. This role offers a pathway to the LPN certificate with up to $20,000 in student loan repayment and includes medical, dental, and vision insurance.
Required Qualifications
- Active Certified Nursing Assistant (CNA) certificate in the state you work
- High School Diploma or certificate
- Minimum one year healthcare experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Must be legally authorized to work in the United States
