Job Summary
Provide personal care and supportive services to hospice patients in their place of residence, including bathing, grooming, dressing, toileting, and mobility assistance. Assist with activities of daily living according to the patient's plan of care while promoting comfort, dignity, and safety. Document patient care and observations accurately, reporting changes in condition to the supervising nurse. Maintain a clean environment, follow infection control procedures, and communicate effectively with patients, families, and the interdisciplinary team. Work independently in a home or facility setting under nurse supervision.
Required Qualifications
- Current state certification as a Certified Nursing Assistant (CNA)
- Reliable transportation
- Valid driver's license
- Current auto liability insurance
- Ability to read, write, and communicate effectively in English
