Job Summary
Hospice Case Manager RN position in Minneapolis with Allina Health. Responsible for comprehensive assessment of patient and family needs, developing and coordinating a client-focused plan of care, and ensuring high-quality home health nursing. Works with care team to screen for healthcare needs, establish measurable outcomes, supervise progress, and modify the plan as needed. Collaborates with family and patient, coordinates and delegates to ensure safe, effective care, and maintains competencies to provide evidence-based nursing practice. Supplemental roles include Charge Nurse and Preceptor as applicable. Requires RN with 2+ years of experience; MN or WI licensure as applicable; BLS certification; driver’s license; and preferred CHPN and CMGT-BC certifications. union-represented position with standard benefits and shift patterns.
Required Qualifications
- Associate's or Vocational degree in nursing
- 2+ years RN experience
- Licensed Registered Nurse – MN Board of Nursing required if working in MN upon hire / WI DSPS required if working in WI upon hire
- Current BLS certification from the American Heart Association OR American Red Cross OR in-house BLS training within 30 days of hire
- Valid Driver’s License required upon hire (MN/WI residents must obtain license in residence within 60 days)
- Nursing Case Management Certification (CMGT-BC) preferred
- Certified Hospice and Palliative Nursing (CHPN) preferred
- Nursing license and meet regulatory requirements for MN/WI
