Job Summary
Hospice RN Case Manager coordinates in-home hospice care with a caseload of ~12-14 patients, delivering care across Gwinnett and adjacent areas. Typical schedule is Monday–Friday, 8am–5pm, with an average of ~4 patients seen per day and responsibility for back-up on-call. Must be a Georgia-licensed RN with 2+ years in-home patient care and preferred hospice/palliative experience; familiarity with EMR KanTime is a plus. The role includes mileage reimbursement and comprehensive benefits (health/dental/vision, 401(k) with match, paid holidays, PTO, life insurance, disability).
Required Qualifications
- Registered Nurse license in Georgia
- 1 year Hospice or Palliative Care experience preferred
- 2 years in-Home patient care experience required
- Familiarity with EMR KanTime preferred
