Hospice Case Manager Registered Nurse

Deliver comprehensive patient-centered nursing care as the primary hospice nurse case manager for Fresno-based clients. Perform patient assessments, develop and update individualized care plans, monitor condition, coordinate with an interdisciplinary team, lead case conferences, supervise LPNs and Hospice Aides, ensure timely documentation and compliance with HIPAA and hospice standards, provide symptom management, and educate patients and families about disease progression, self-care, and end-of-life expectations. Travel to patient homes is required; role emphasizes care coordination, teaching, and compassionate, dignified end-of-life support.
