Job Summary
Hospice RN Case Manager responsible for maintaining a caseload of about 12-14 patients across the Decatur area and surrounding communities. Office hours are Monday–Friday, 8am–5pm, with on-call responsibilities for back-up coverage. The role requires a Georgia Registered Nurse license and experience in hospice/palliative care (1 year preferred) and in-home patient care (2 years required). EMR experience with KanTime preferred. Benefits include salary in the $85k–$93k range, employer-paid health/dental/vision insurance, mileage reimbursement, PTO, holidays, 401(k) with match, and other supplemental coverage. Locations include Decatur, GA and nearby areas (Decatur, Sandy Springs, Dunwoody, Peachtree Corners, Norcross).
Required Qualifications
- Registered Nurse in the State of Georgia
- 1 year of Hospice or Palliative Care Experience-Preferred
- 2 years of in-Home Patient Care Experience-Required
- Familiarity with EMR System KanTime-Preferred
