Hospice Care Registered Nurse, Westchester

Job Summary
Performs an age-specific plan of care for a designated group of patients using the nursing process (assessment, diagnosis, planning, implementation, evaluation). Collaborates with physicians and the health team to coordinate and implement procedures and treatments; uses leadership and clinical judgment to coordinate patient care and direct/delegate activities of the patient care unit team. Conducts pre-admission screening and initial nursing health assessment; develops nursing plan of care and goals; provides nursing care including supervision of diet, medications, activity level, safety in the home, and activities of daily living. Evaluates nursing care and coordinates with therapy services; trains home health aides and personal care workers; determines equipment needs; participates in on-call coverage; contributes to Quality Improvement. Field position in Westchester/ NY; on-site care with potential travel to patient locations.
Required Qualifications
- Graduate from an accredited School of Nursing
- Bachelor’s Degree in Nursing, preferred
- Current License to practice as a Registered Professional Nurse in New York State
- Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date
- 2+ years of nursing experience: med/surg, hospice, home care preferred
- Minimum one year of experience in acute care – med/surg strongly preferred
- Experience in inpatient facilities and hospice care
- This is a Field position; may need to see Pediatric patients
