Job Summary
Hospice Care Registered Nurse in the field for Queens provides in-home nursing care, conducts pre-admission assessments, and develops and implements patient care plans using the nursing process. Collaborates with physicians and the Interdisciplinary Group to coordinate procedures and treatments, and exercises leadership/clinical judgment to direct activities of the patient care unit team. Responsibilities include supervising and training home health aides, evaluating equipment and supplies, coordinating referrals for therapy and nutrition services, participating in on-call coverage, contributing to quality improvement, and ensuring safe home environments for patients. Requires NY RN licensure, BSN preferred (enrollment/degree timeline as specified), and 2+ years of nursing experience with hospice/home care background.
Required Qualifications
- Graduate from an accredited School of Nursing
- Bachelor’s Degree in Nursing, preferred
- Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date
- Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed
- 2+ years of nursing experience: med/surg, hospice, home care preferred
- Minimum one year of experience in acute care – med/surg strongly preferred
- Experience in community health, hospice, and home care preferred
- This is a Field position.
