Job Summary
The Hospice Care Registered Nurse performs patient assessments and develops individualized nursing care plans. Collaborates with interdisciplinary teams to provide nursing care in patient homes, including training home health aides. The role requires current New York RN licensure and at least two years of nursing experience, preferably in hospice or home care settings.
Required Qualifications
- Graduate from an accredited School of Nursing
- Current License to practice as a Registered Professional Nurse in New York State required
- 2+ years of nursing experience: med/surge, hospice, home care preferred
- Minimum one year of experience in acute care – med/surg strongly preferred
- Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date
