Job Summary
Hospice Care Registered Nurse – Fee for Service. Performs age-specific nursing care planning and execution for designated patients using the nursing process, and collaborates with physicians and the health team to coordinate procedures and treatments. Responsibilities include conducting pre-admission screening and health assessments, developing and implementing nursing care plans, supervising diet, medications, activity level, and safety in the home, and evaluating the effectiveness of care with multiple therapies. Trains and evaluates home health aides and personal care workers, coordinates equipment needs, participates in Interdisciplinary Group decisions, and supports on-call coverage and quality improvement activities. Requires a NY RN license with BSN preferred, and 2+ years of nursing experience (hospice/home care preferred); this is a field position serving patients in Riverhead, NY. The role may include an orientation and administrative tasks paid at $30/hour.
Required Qualifications
- Graduate from an accredited School of Nursing
- Current license to practice as a Registered Professional Nurse in New York State
- Bachelor’s Degree in Nursing preferred
- 2+ years of nursing experience (med/surg, hospice, home care preferred)
- Minimum one year of experience in acute care – med/surg strongly preferred
- Enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date
