Job Summary
Hospice Care Registered Nurse provides age-appropriate nursing care plans, assessment, and coordination with physicians and health-team members. Responsibilities include pre-admission screening, nursing health assessments, developing and evaluating care plans, supervising home health aides and personal care workers, recommending equipment, coordinating therapies, participating in on-call coverage, and contributing to Quality Improvement efforts. Requires NY RN license, BSN enrollment with target degree within five years, 2+ years of nursing experience (prefer hospice/home care and med/surg), and field-position duties.
Required Qualifications
- Graduate from an accredited School of Nursing
- Bachelor’s Degree in Nursing (preferred)
- Current License to practice as a Registered Professional Nurse in New York State
- 2+ years of nursing experience (med/surge, hospice, home care preferred)
- Minimum one year of acute care experience (med/surg strongly preferred)
- Experience in inpatient facilities and hospice care
- Must be enrolled in an accredited BSN program within two years and obtain a BSN Degree within five years of job entry date
