Job Summary
Hospice Care Planner responsible for evaluating and establishing a care plan for Hospice patients referred from hospital to appropriate Hospice programs; facilitates admissions; develops and maintains relationships with physicians, social workers, case managers and discharge planners; requires NYS RN licensure, with at least two years Hospice nursing experience, strong assessment and interview skills, client-service orientation, excellent communication, advanced critical thinking, and ability to work independently; bilingual Spanish/English preferred in some locations. On-site at a major medical center in White Plains, NY.
Required Qualifications
- Licensed Registered Professional Nurse in NYS is required
- Minimum two years experience in Hospice nursing with proficiency in physical assessment and interviewing skills
- Must be client-service focused with excellent written and verbal communication skills
- Advanced critical thinking and problem solving skills, plus the ability to work independently
- Bilingual Spanish/English is required in some locations
