Job Summary
Hospice Care Planner position based at a major medical center; on-site role responsible for evaluating and establishing a plan of care for patients referred for hospice services, facilitating admission of referrals from hospital to appropriate hospice program, and developing positive relationships with physicians, social workers, case managers, and discharge planners. Requires current NYS RN license and at least two years of hospice nursing experience; strong assessment, interviewing, communication skills, critical thinking, and independence. Bilingual Spanish/English is required in some locations. Located in Ridgefield Park, NJ with on-site work expectation.
Required Qualifications
- RN license in NYS
