Hospice Care Planner

Job Summary
Evaluate and establish comprehensive plans of care for patients referred for Hospice Care services. Facilitate admission of referrals upon hospital discharge by assessing patient and family needs. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center. Requires current NYS Registered Professional Nurse licensure, two years of Hospice nursing experience, and proficiency in physical assessment and interviewing skills. Bilingual Spanish/English capability is required in some locations. As a key member of the business development and marketing team, this role supports our groundbreaking in-patient and community programs designed to ease the burden of life-limiting illness.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Proficiency in interviewing skills
- Client-service focused
- Excellent written and verbal communication skills
- Advanced critical thinking
- Advanced problem solving skills
- Ability to work independently
- Bilingual Spanish/English (in some locations)
