Job Summary
Evaluate and establish comprehensive plans of care for patients referred for Hospice Care services, facilitating their admission to the program upon hospital discharge. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center. Conduct physical assessments and utilize advanced critical thinking to solve complex patient needs while working independently. Requires current NYS Registered Professional Nurse licensure, two years of Hospice nursing experience, and bilingual Spanish/English proficiency in some locations. As a key member of the business development and marketing team, this role supports our groundbreaking programs for easing the burden of life-limiting illness and end-of-life care.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Proficiency in interviewing skills
- Client-service focused
- Excellent written and verbal communication skills
- Advanced critical thinking
- Advanced problem solving skills
- Ability to work independently
- Bilingual Spanish/English (in some locations)
