Job Summary
Evaluate and establish a plan of care for patients referred for Hospice Care services, facilitating their admission upon discharge from the hospital to a program that meets patient and family needs. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center as a key member of the business development and marketing team. Requires current Registration to practice as a Registered Professional Nurse in NYS, a Bachelor's degree preferred, and minimum two years of Hospice nursing experience with proficiency in physical assessment and interviewing skills. Bilingual Spanish/English required in some locations.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Interviewing skills
- Client-service focused
- Excellent written and verbal communication skills
- Advanced critical thinking
