Job Summary
Evaluate and establish comprehensive plans of care for patients referred to Hospice services upon discharge from the hospital. Facilitate admissions by meeting the specific needs of patients and their families while developing and maintaining positive relationships with physicians, social workers, case managers, and discharge planners. Conduct physical assessments and interviews to ensure effective client service, utilizing advanced critical thinking and problem-solving skills to work independently. Maintain current New York State Registered Professional Nurse licensure with a minimum of two years in Hospice nursing. Bilingual Spanish/English capability is required in some locations. As a key member of the business development and marketing team, this role supports our groundbreaking in-patient and community programs.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Proficiency in interviewing skills
- Client-service focused
- Excellent written and verbal communication skills
- Advanced critical thinking
- Problem solving skills
- Ability to work independently
- Bilingual Spanish/English (in some locations)
