Job Summary
Evaluate and establish comprehensive plans of care for patients referred for Hospice Care services upon discharge from the hospital. Facilitate admission to programs that meet the specific needs of patients and their families. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center. Requires current Registration to practice as a Registered Professional Nurse in NYS, a Bachelor's degree, and minimum two years of Hospice nursing experience. Bilingual Spanish/English proficiency is required in some locations. This role supports our groundbreaking in-patient and community programs designed to ease the burden of managing life-limiting illnesses.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Proficiency in interviewing skills
- Client-service focused
- Excellent written and verbal communication skills
- Advanced critical thinking
- Advanced problem solving skills
- Ability to work independently
- Bilingual Spanish/English (in some locations)
