Job Summary
Evaluate and establish comprehensive plans of care for patients referred for Hospice Care services, facilitating their admission from hospital discharge to in-patient facilities or community programs. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center to ensure seamless referrals. Requires current NYS Registered Professional Nurse licensure, two years of Hospice nursing experience, and proficiency in physical assessment and interviewing skills. Bilingual Spanish/English capability is required in some locations. As a key member of the business development and marketing team, this role supports our groundbreaking programs that ease the burden of end-of-life care for patients and their families.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Proficiency in interviewing skills
- Client-service focused
- Excellent written communication skills
- Excellent verbal communication skills
- Advanced critical thinking skills
- Problem solving skills
- Ability to work independently
- Bilingual Spanish/English (in some locations)
