Job Summary
Evaluate and establish comprehensive plans of care for patients referred for Hospice services, facilitating their admission to the program upon discharge from the hospital. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center to ensure seamless transitions. This role requires a Licensed Registered Professional Nurse in NYS with at least two years of Hospice nursing experience, proficiency in physical assessment and interviewing, and bilingual Spanish/English capability in certain locations. As a key member of the business development and marketing team, you will apply advanced critical thinking and problem-solving skills to work independently while prioritizing client service and clear communication.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Interviewing skills
- Client-service focused
- Excellent written and verbal communication skills
