Hospice Care Planner

Job Summary
Evaluate and establish comprehensive plans of care for patients referred for hospice services upon discharge from the hospital. Facilitate admission to hospice programs that meet the specific needs of patients and their families. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center. Requires current NYS Registered Professional Nurse licensure, a minimum of two years in hospice nursing, and proficiency in physical assessment and interviewing skills. Bilingual Spanish/English capability is required in some locations. As a key member of the business development and marketing team, this role supports the organization's groundbreaking programs offering in-patient and community services to ease the burden of life-limiting illness.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Proficiency in interviewing skills
- Client-service focused
- Excellent written and verbal communication skills
- Advanced critical thinking
- Advanced problem solving skills
- Ability to work independently
- Bilingual Spanish/English (in some locations)
