Hospice Care Planner

Evaluate and establish comprehensive plans of care for patients referred for Hospice Care services upon discharge from the hospital. Facilitate admission to appropriate Hospice programs that meet the specific needs of patients and their families. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center. Requires current Registration to practice as a Registered Professional Nurse in NYS, a Bachelor's degree preferred, and minimum two years of Hospice nursing experience with proficiency in physical assessment and interviewing skills. Bilingual Spanish/English capability is required in some locations.
