Job Summary
Evaluate and establish care plans for patients referred to Hospice services upon hospital discharge. Facilitate admission to programs that meet patient and family needs while developing relationships with physicians, social workers, case managers, and discharge planners. Maintain positive connections within the business development and marketing team to support in-patient and community programs. Requires current NYS Registered Professional Nurse licenship, two years of Hospice nursing experience, and proficiency in physical assessment and interviewing. Bilingual Spanish/English capability is required in some locations.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Proficiency in interviewing skills
- Client-service focused
- Excellent written and verbal communication skills
- Advanced critical thinking
- Advanced problem solving skills
- Ability to work independently
- Bilingual Spanish/English (in some locations)
