Job Summary
Evaluate and establish comprehensive plans of care for patients referred for Hospice Care services upon discharge from the hospital. Facilitate admissions to the Hospice program by meeting the specific needs of patients and their families. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center as a key member of the business development and marketing team. Requires current Registration to practice as a Registered Professional Nurse in NYS, a Bachelor's Degree, and a minimum of two years of Hospice nursing experience. Bilingual Spanish/English proficiency is required in some locations.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Proficiency in interviewing skills
- Client-service focused
- Excellent written communication skills
- Excellent verbal communication skills
- Advanced critical thinking
- Advanced problem solving skills
- Ability to work independently
- Bilingual Spanish/English (in some locations)
