Job Summary
Evaluate and establish comprehensive plans of care for patients referred for Hospice Care services upon discharge from the hospital. Facilitate admission to the Hospice program by meeting the specific needs of patients and their families. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center. This role requires a Licensed Registered Professional Nurse in NYS with at least two years of Hospice nursing experience, proficiency in physical assessment and interviewing skills, and bilingual Spanish/English capability in some locations. As a key member of the business development and marketing team, you will apply advanced critical thinking and problem-solving skills to work independently in supporting end-of-life care programs.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Proficiency in interviewing skills
- Client-service focused
- Excellent written communication skills
- Excellent verbal communication skills
- Advanced critical thinking skills
- Advanced problem solving skills
- Ability to work independently
- Bilingual Spanish/English (in some locations)
