Hospice Care Planner

Job Summary
Evaluate and establish comprehensive plans of care for patients referred for Hospice Care services. Facilitate admission of referrals upon discharge from the hospital to a Hospice program that meets the specific needs of patients and their families. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center as a key member of the business development and marketing team. This role requires a Registered Professional Nurse in NYS with at least two years of Hospice nursing experience, proficiency in physical assessment and interviewing skills, and bilingual Spanish/English capability in some locations.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Proficiency in interviewing skills
- Client-service focused
- Excellent written communication skills
- Excellent verbal communication skills
- Advanced critical thinking skills
- Advanced problem solving skills
- Ability to work independently
- Bilingual Spanish/English (in some locations)
