Job Summary
Evaluate and establish comprehensive care plans for patients referred to Hospice services upon hospital discharge. Facilitate admissions to the Hospice program while developing and maintaining positive relationships with physicians, social workers, case managers, and discharge planners. Conduct physical assessments and interviews to ensure the plan meets the specific needs of patients and their families. Requires current NYS Registered Professional Nurse licensure, two years of Hospice nursing experience, and bilingual Spanish/English proficiency in some locations.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Proficiency in interviewing skills
- Client-service focused
- Excellent written communication skills
- Excellent verbal communication skills
- Advanced critical thinking
- Advanced problem solving skills
- Ability to work independently
- Bilingual Spanish/English (in some locations)
