Hospice Care Planner

Evaluate and establish comprehensive plans of care for patients referred to Hospice services upon hospital discharge. Facilitate admissions while maintaining positive relationships with physicians, social workers, case managers, and discharge planners to ensure patient and family needs are met. Conduct physical assessments and interviews, leveraging advanced critical thinking to solve complex end-of-life care challenges independently. Requires current NYS Registered Professional Nurse licensure, two years of Hospice nursing experience, and bilingual Spanish/English proficiency in select locations.
