Job Summary
Evaluate and establish a plan of care for patients referred for Hospice Care services, facilitating admission from hospital discharge to the program. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center. Requires current Registration to practice as a Registered Professional Nurse in NYS, a Bachelor's Degree, and a minimum of two years in Hospice nursing with proficiency in physical assessment and interviewing. Bilingual Spanish/English is required in some locations. As a key member of the business development and marketing team, this role supports our groundbreaking programs offering in-patient and community services to ease the burden of life-limiting illness and end-of-life care.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Interviewing skills
- Client-service focused
- Excellent written and verbal communication skills
- Advanced critical thinking
- Problem solving skills
- Ability to work independently
