Hospice Care Planner

Evaluate and establish a plan of care for patients referred for Hospice Care services and facilitate their admission to the program upon hospital discharge. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center as a key member of the business development and marketing team. Requires current Registration to practice as a Registered Professional Nurse in NYS, a Bachelor's degree, and at least two years of hospice nursing experience with proficiency in physical assessment and interviewing skills. Bilingual Spanish/English is required in some locations. This role supports our groundbreaking in-patient and community programs designed to ease the burden of managing life-limiting illnesses for patients and their families.
