Job Summary
Evaluate and establish comprehensive care plans for patients referred for Hospice and Palliative services upon discharge from the hospital. Facilitate admission to a Hospice program that meets the specific needs of patients and their families. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center. This role requires current NYS Registered Professional Nurse licenship, two years of Hospice nursing experience, and proficiency in physical assessment and interviewing skills. Bilingual Spanish/English capability is required in some locations. As a key member of the business development and marketing team, you will apply advanced critical thinking to manage end-of-life care transitions.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Interviewing skills
- Client-service focused
- Excellent written and verbal communication skills
- Advanced critical thinking
- Problem solving skills
- Ability to work independently
- Bilingual Spanish/English (in some locations)
