Hospice Care Planner

Evaluate and establish a plan of care for patients referred for Hospice Care services, facilitating admission from hospital discharge to the program. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center. Requires current Registration to practice as a Registered Professional Nurse in NYS, a Bachelor's Degree preferred, and minimum two years of Hospice nursing experience with proficiency in physical assessment and interviewing skills. Bilingual Spanish/English is required in some locations. As a key member of the business development and marketing team, this role focuses on easing the burden of managing life-limiting illness for patients and their families.
