Hospice Care Planner

Job Summary
Evaluate and establish comprehensive plans of care for patients referred for Hospice Care services. Facilitate admission of referrals from hospital discharge to the Hospice program, ensuring alignment with patient and family needs. Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center. Maintain current Registration to practice as a Registered Professional Nurse in NYS and possess minimum two years of Hospice nursing experience. Bilingual Spanish/English proficiency is required in some locations. As a key member of the business development and marketing team, support the organization's groundbreaking programs in both in-patient facilities and the community.
Required Qualifications
- Licensed with current Registration to practice as Registered Professional Nurse in NYS
- Minimum of two years experience in Hospice nursing
- Proficiency in physical assessment
- Proficiency in interviewing skills
- Client-service focused
- Excellent written and verbal communication skills
- Advanced critical thinking
- Advanced problem solving skills
- Ability to work independently
- Bilingual Spanish/English (in some locations)
