Job Summary
Care Coordinator in hospice responsible for generating referrals, growing professional relationships with physicians, nursing homes, hospitals, discharge planners, and case managers; manage a territory in Columbus, OH; deliver educational in-services, implement marketing initiatives, document activities, and collaborate with the Patient Care Manager to ensure seamless patient transitions; participate in evening and weekend on-call admissions; travel within the local service area and represent Transitions Hospice across community interactions.
Required Qualifications
- Bachelor’s degree and relevant healthcare experience or knowledge preferred
- 3+ years of sales experience, ideally in a healthcare setting
- A demonstrated track record of building meaningful customer relationships that drive results
- Excellent communication, negotiation, and public relations skills
- Ability to market effectively to referral sources, customers, and the broader community
- Comfort working within an interdisciplinary team environment
- Strong organizational skills and the ability to manage a territory independently
- Valid driver’s license and willingness to travel within your local service area
- Professional appearance and demeanor at all times
