Job Summary
Hospice Care Coordinator coordinates ancillary services for hospice patients under the supervision of the Hospice Team Manager. Responsibilities include ordering lab tests and medical supplies, documenting in the EMR, faxing lab results to the patient’s PCP, communicating with patients and families about equipment delivery, processing admissions and discharges, preparing agendas for Interdisciplinary Team (IDT) meetings and attending as a scribe, supporting the Team Manager and IDT members, and preparing patients and families for discharge and follow-up care. Essential functions align with ADA guidance and related duties.
Required Qualifications
- High School Diploma or equivalent
- 1-3 years of relevant experience
