Hospice Care Consultant

Job Summary
Interact with healthcare professionals in assisted living communities to coordinate admissions based on clinical criteria and review patient referrals to discuss hospice care options. Develop and execute marketing plans while providing education and in-services to referral sources. Participate in committees, special projects, and related duties to support patient transitions and care planning. This role requires a Bachelor's degree or three years of healthcare experience and proficiency in Windows, Excel, and Word. Lifesong Hospice and Palliative Care provides a locally owned environment with on-staff clinical mentors and a dedicated weekend and evening on-call team.
Required Qualifications
- Must have prior sales and marketing experience within the healthcare industry
- Bachelor's Degree in a related field
- 3 years previous healthcare experience in hospice, home health, healthcare sales or marketing
- Excellent verbal and written communication skills
- Demonstrate an ability to work closely with a variety of people while effectively using problem solving skills
- A willingness to learn Medicare regulations and related reimbursement issues for hospice and ALF/PC environments
- Proficiency/Knowledge of Windows, Excel and Word
- Demonstrate willingness to work as a team player within the department and with the clinical and operational teams
Desired Qualifications
- Prior Hospice specific experience is preferred
