Hospice Care Consultant

Drive significant growth in hospice admissions within a defined book of business by opening new accounts and expanding existing ones; secure referrals from physicians, staff, and administrators and partner with operations to rapidly admit clinically eligible patients; own and exceed weekly performance goals with tenure-based expectations; build and maintain trust-based relationships with referral sources using a consultative sales approach; demonstrate compassion and empathy to patients and families, and be willing to work outside standard hours (evenings and weekends) to ensure timely support and service; provide clinical training to referral partners to improve understanding of hospice care; actively adapt to changing priorities and drive improvements in service delivery.
