Job Summary
Hospice Care, Bereavement Counselor (LCSW/LMSW) role providing counseling to bereaved family members of hospice patients and the community. Responsibilities include assessing bereaved clients, orienting them to programs, collaborating with healthcare team to develop client plans of care, conducting individual/group/family counseling, and serving as a liaison to referral sources to facilitate continuity of care. Requires a Master’s Degree and active NY licensed social work or mental health counseling licensure, with 1–3 years of technical experience and bereavement/grief counseling background preferred. Must operate within the scope of duties under the Americans with Disabilities Act and may involve related duties as assigned.
Required Qualifications
- Master's Degree required
- Current License to practice as a Master Social Worker (LCSW) or LMSW in New York State, or Current License to practice as a Mental Health Counselor in New York State
- 1-3 years of technical experience required
- Minimum of 1 year of bereavement and grief counseling experience strongly preferred
- Experience with individual and group sessions and assessment
- Experience working with all ages and families preferred
- Hospice care, home care, or community health experience preferred
